Thrive Parent Contract
Contract is from August 2024- May 2025
PARENTS: Please read and then complete the form on this page.
1) Parent agrees to the educational services indicated BELOW and understands this contract is based on a yearly basis. Days missed for inclement weather, sickness, holidays, or parental choices will not be reimbursed.
2) Payment is due on the 15th of each month, acting as a pre-payment for services for the month. Invoices will be emailed on the first of the month.
3) If payments are not received by the 15th of each month, student will ineligible for services until payment is made and a $40 late fee per month after the 15th of the month.
4) Cessation of services before end of contract year will result in a 35% fee of remainder of year’s tuition unless unforeseen circumstances arise as determined by director.
5) Parent or guardian agrees to indemnify and hold harmless THRIVE for any liability resulting from personal injury, property damage, or monetary loss arising out of or as a result of services rendered through THRIVE.
6) This contract creates a legal obligation to pay for services rendered through THRIVE. The person signing the contract assumes financial responsibility for services provided through THRIVE.
7) Parent understands this contract is between THRIVE and the undersigned.
8) Parent understands by signing this document they are committing to the 2024-2025 Academic Year beginning August 2024 and ending May 2025.
NCSEAA Grant Recipients: The state of North Carolina issues grant money for children with a diagnosed learning disability. You may apply for this grant at their website beginning February 1, 2024 with priority enrollment through March 1, 2024. If funds are available after March 1, 2024, applications will still be accepted. www.ncseaa.edu Grant recipients will be responsible for any remaining charges that grant funds do not cover and are subject to a late fee. Grant recipients are responsible for submitting invoices to NCSEAA for payment so that payments can be made on time. If request for payment is not made in time for the 15th due date to be met, late fees will be charged. A total amount of $9000-$17,000 can be available per student per academic year from NCSEAA
